You MUST make sure when you fill out any forms at the doctor’s office that you write on the form: THIS WAS A WORKERS’ COMPENSATION INJURY.

If you know who the insurance company is, write down the name of the insurance company.

If you know what the claim number is, write down the claim number also.

Of course, if you have any contact information for the insurance company, write that down also.

I cannot tell you how many times I’ve seen people fail to write this down.

If you fail to do this, it WILL blow up on you later.
